Thank you for considering our foundation to assist you with your pets needs. Please read the guidelines and make sure to have all appropriate documentation before clicking on the Application for Veterinary Funds. While it is our goal to help every dog in need, please understand funding is provided on a case by case basis. Once your application and required documents are complete please email them to us at applications@dogsandadvicefoundation.com. If you are unable to scan or take a picture and email complete application, please mail to the P.O. Box provided and send an email to let us know your application is in transit.

To apply for veterinary financial aid please make sure you have the following paperwork as it will be necessary for the application and approval process.

Please note D&OUA Foundation will not provide financial assistance for routine veterinary care such as vaccinations, spay/neuter and dental procedures please contact your local humane society for assistance with these procedures. If these are scheduled with an emergency surgery funding may be denied.

 

  • Estimate for procedures needed. This must be provided by your vet on their letterhead and will be verified with them. Funds will be issued directly to the veterinary practice confirmed on the estimate, at no time will funds be paid out to the applicant or other individual.

  • Copy of drivers license or state issued ID card and a copy of your most recent utility bill. We will use this to confirm the identity of the owner. At no time will your personal information be shared with any other entity.

  • Proof of application and denial from Care Credit(https://www.carecredit.com/vetmed/)

  • A letter from the applicant introducing yourself and your pet and why you need financial aid.

  • Applicant must be willing to provide photos and updates and attend all follow up appointments as determined by the veterinarian listed on the estimate.

*In an effort to impact as many families as possible, partial financial aid is a possibility, full coverage of a veterinary estimate is not guaranteed. In the instance of partial aid, the owner WILL BE responsible for the remaining balance due to their veterinarian. This decision will be based on several factors such as urgency, financial need, available funding and eligibility. Completion of application is not a guarantee of aid; applicant will be notified of funding status by email within 24-48 hours depending on completion status. The amount of funding approved is subject to change at any time prior to payment. Incomplete applications will not be considered *
